1. Complete the Distance Learning
Correspondence Testing Student Request Form. Request this form at:

2. Once Teach Enrich Testing Center receives
your request, a team member will contact
you within 2-3 business days to confirm
whether we are able to accommodate your
request. At that time, you will receive
payment instructions as well as
confirmation of your appointment details.

3. Notify your home institution that you've
selected a testing center and obtain
approval to test at PVU Teach Enrich
Testing Center. Exam materials must be
submitted via one of the following
methods: email attachment, fax, mail, or
overnight courier. This information can be
found above in the yellow section.

All test takers must present one (1) primary form of government-issued identification that includes their photograph and signature. According to the rules of your specific exam, you may be asked to present a second form of ID. Below are acceptable forms of ID.
ACCEPTABLE ID

1. Government-issued passport with name, photograph & signature.
2. Driver's license with name, photograph & signature.
3. State or Province ID issued by motor vehicle agency with name,
photograph & signature.
4. Military ID with name, photograph & electronic signature.
5. National ID with name, photograph & signature.
6. Tribal ID card with name, photograph, & signature.
7. A naturalization card or certificate of citizenship with name, photograph &
signature.
8. A Permanent Resident Card (Green Card) with name, photograph &
signature.

A second form of ID may be requested. This second ID must have the test taker's name with a signature OR a photograph. Below are some valid forms of a secondary ID:

1. School ID 2. Debit or Credit Card
3. Social Security Card 4. Voter Registration Card

6. Other official ID with the test taker's name & signature.
